  1. login to your Supervisor
  2. open a Terminal window
  3. run the following command
"$QBDIR/utils/upgrade_supervisor" -reset

where $QBDIR is the location of your Qube installation: /Applications/pfx/qube on OSX, c:\Program Files\pfx\qube on Windows, or /usr/local/pfx/qube on OSX.
